Please print the form and then fill in your information. When you have completed the form, scan it as a PDF, and then go back to the website and look under Contact Us for “Upload Documents”. Select this option and then you may drag and drop the form or select the form from your computer to upload it to the Doctor’s Office.
Alternatively, you may bring the form in with you when you come to visit.
Doing this will speed up your visit on the day of your appointment.
